Here is a simple CCF that contains all the IR codes. It features an IR layer with all the DVD player's controls, plus two panels with buttons aliased to the IR panel. Good and simple, but functional at the same time.

Updated with Blue & Yellow codes that match the PS3IR-1000 firmware v1.04.
